What do you think about using your keyword or keyprase twice in your title?
Have you seen a difference on performance?

No difference on performance. Moreover,using twice keywords in tags could be read as a spam issue by SEs. Don't focus on the title tag.work well on the body text.

Yes,of course,but they recognize keyword repeating and could penalize you. You should find a different solution to increase kw density in title tag.

I see lots of repeated keywords in top ten positions every day, don't know if that "penalization" is real

Speaking just as a searcher, if I saw repeated words in a title (when displayed in the SERPs) I would think, "Probably a junk spam site" and go on to the next one.

I don't because I know everybody wants to be on top and title has nothing to do with content inside. In the other hand may correct syntax and good titles repeats the keyword in a useful way.

I recently changed the title to add my target keyword a second time and dropped 60 places on Google. Assume I was penalised for this? Any idea how long it might take to get it back?

Simply remove your redundant keywords or change it's order if that was the reason of course. Consider other factors, think what has changed.
