Is it better to focus on a "keyword" or a "keyword phrase". For instance, "my_keyword" or "my_keyword and my_keyword2"?
For keywords that are a bit difficult to rank up high with competition, I wonder if focusing on more localized descriptions, like "my_keyword state" would work better?
Any thoughts?

Hi, Jeremy. Definately focus on multiple KW phrases. Two-threes KW phrases, sometimes even more depending on what industry you're in. And yes! Adding a location keyword such as state, city, etc. can be very effective. Again, depending on your industry and the users you're targeting.

You might also want to review Michael Campbell's "low hanging fruit" theory. Do some searches and you should be able to find it. If nothing else, it is an interesting read.
