Javascript Auto-Fill for forms

I have a form that has a main section that asks for name, address, phone etc.

The next section asks for the same information(name, address, phone etc.) for different people.

How can I get it so I can have a checkbox that says "Click here if your address is the same as above" and populate the above address information into the new section.

There will be 4 different instances of this, for 4 different people.

try this one from javascript.internet.com [javascript.internet.com] which allows you to copy fields if they are the same
