Crazy form positioning

I'm working on a form for a shopping cart (ProfCS) email list. However, the text is positioning itself very wierdly on firefox (it's fine in IE).

I'd be extremely grateful if people could check it out and let me know why this is happening. The form is at <snip> - please just click 'Submit Query' on the left to see the form - the problem is in the name and email fields (you don't need to change the default email address/name).

I did have a look at your site nevertheless, using firefox, and I couldn't see anything strange about the form display.

How odd - maybe it's just my version of firefox. Will try on others.
