what is the easiest way to make a small exit popup with a short description in it with a subscription form in it

How about using onunload [w3.org] to fire a script that launches a popup window with the form on it?

One caveat though, this will irritate a lot of people!

IMHO it will so hugely incense the majority of visitors that they won't go near the site again.

One thing to consider -- you probably don't want to server the pop-up every time someone exits a page on your domain, but only when they are going to a completely different domain and leaving you. Here's a discussion about that:
