my site ranked #4 last months,but now it down to #60,i had read almost all article for it,but i has not found any unquestionable news,i has no hidden text ,cloaking,spaming and so on tricks,but there are some hidden input form text in it,i do not know if my site was punished for it? thanks in advance please help me! I don't think that a <input type=hidden ...> tag has caused the drop. I would look for other reasons, e.g. backlinks are lost, change in the ranking algorithm, ...

No way they would ever punish for this; they'd wipe out half of the index including themselves. I have hidden form elements all over many sites as I'm sure most sites that use forms do as well.
Now if you had 300 hidden form elements stuffed with keywords that didn't insert into the form/DB, maybe you'd incur a penalty from a human review, but I'd guess Googlebot would simply ignore it.
