If your site was penalize and then promised to be good again then do the necessary update to your site, how do you get it again? Should you just wait for another crawl or send them an email for a request?

I would do both, [make necessary changes and] be patient and also send them a really nice, short, and concise email explaining your situation and the changes you made - Re: Reinclusion Request with your WW Nick. You may also want to create an additional contingency plan by preparing a replacement site on a new URL (discretionary based upon the nature of your violation and current SEO tactics).
