Recommend a page on a site - need some pointers

I am looking for a form which I can use to submit to my chosen email system that pulls that URL of the page it's on. (Tell a friend type thing).

It needs to be in ASP or Javascript and is going to be used in server side includes throughout the site.

No matter how hard I look, I can only find PHP or CGI versions.
It would need to have a field for your name, your emails, send to email and comments. I can add validation.

It must be simple but as a newbie Javascript programmer it's beyond me at the moment.

Can anyone point me to a site or help?

I would love to recommend some really nice scripts in PHP and Perl, but I guess, you have your heart set on ASP and Java only.
