How do you get an icon in the address bar of a browser? And what size and format does the image have to be?

Do a search for favicon and you will find everything you need to know.

I make my own in Phototshop, but htmlkit.com has a generator that creates favicons from images. Useful for people that just want one or two, and want to be done with it.

Why does my favicon only show up in Mozilla Firefox?

Explorer shows the favicon only on a bookmark. But Opera and Safari also show it in the address bar. So it's not a standard thing at all, it's specific to each browser.

Not even sure, that is correct. Man, the IE-way for favicon usage is strange as far as I'm concerned. It seems that when you re-visit a bookmarked page IE no longer looks for the favicon... I just can't figure out the peculiarities with the IE usage. IMHO

(The only reason I use a favicon is to keep the gazillion 'file not found' out of my server logs!)
