I found one from January from a little-used IP (also some earlier blocked requests from 54.blahblah for an interior page):

67.221.174.242 - - [11/Jan/2016:19:51:34 -0800] "GET / HTTP/1.1" 200 5639 "-" "Dispatch/0.11.1-SNAPSHOT"

200 because I never got around to blocking the range. Further detour to header logs reveals two separate offenses.
