Optimizing for an Abbreviation

Some interesting search engine results.

I'll try to do this without being specific enough to get snipped,

My main KW phrase contains an abreviation, looking at O's search tool, adwords results etc., I know that searchers almost always abreviate, so lets say my main KW1 of 3 is doctor thus all searches = dr (this is not my actual kw)
KW2 is a name, I'll use moishe here
KW3 is an abreviated place that is quite common, for this I'll use usa, so people most often search:
dr moishe usa
I have optimized my page for this, worked on getting (now 100) relevent PR3+ backlinks (PR3 for page I am actually on) with kw1 kw2 kw3 as the anchor text.
search major search engines and nothing....ahh, but wait---search for kw2 kw3 and bingo, #3 spot on king G's search index.
This is a problem as most people include kw1 when searching, I never have kw2 on my page without kw1 before it. Both searches bring relevent results for the place being searched for, so why would I not rank with the 3 word combo, but do well for the 2nd and 3rd word combo?
Any ideas anyone?

You didn't mention it, but let me guess: your title is kw1 kw2 kw3 . Might be too much of a coincidence. Try either to vary anchor texts (difficult, I guess) or play with your title tag a bit.

Yes my title contains kw1 kw2 kw3 on some of my pages and my inbound links are all "word word word kw1 kw2 kw3", it almost as though G is only picking up the last two words of my inbound links.

an update, I am now the #2 spot in G for my useless kw2 kw3 combo, I think I am going to rebuild the site so that all of my pages are mydomain.com/kw1-kw2-otherkw.html, I currently us underscores to name files and I guess this was a big mistake.
