Will nofollow keep me safe with Google?

I have a Moving Company in Canada and can't handle USA to USA moves. So this large American website has provided me with a form for my users to use. I make money per leads.

I want to put a banner on my site that will lead to that "affiliate" page. I'll do this manually and add the noindex, nofollow tag.

Will this protect me 100% from Google not penalizing me?

There's no penalty for offering your affiliate link whether it is nofollowed or not. It's only one link. If that webpage has similar content to yours you may want to consider allowing the spider a free crawl to it in order to try and gain some outbound link relevancy points for your webpage.

But actually...

When I had the form from this large moving site as an IFRAME Joomla wrapper on my index file, my site took a fantastic plunge from nearly 100 uniques a day to zero for days on end.

Then I removed the form and the traffic is back to normal again...

That site I am referring to is <snip>, not a spammy site at all.

So I really wonder why I took such a ranking dive?

I want to put a banner on my index file to my affiliate fill a form for estimate page on their site, all relevant to my industry also, will the nofollow keep me 100% safe from whatever this cause before?

Google does not penalized to placing affiliate link or banner on site. Suddenly going down your traffic after placing affiliate page is not actual reason. You should check visitors bounce rate for it.
