Usually just the homepage, be sure to have links to subpages.
Free submission to Google (if you have incomming links, googlebot will usually find your site by it self)
Depending on language, target countries, competition, etc. paid submission to FAST and especially Inktomi is good.
Paid submission to Yahoo - again, be sure your site will pass the editor test it's $299 NONREFUNDABLE.
Allways hand submit, DON'T use autosubmissions, bulksumbissions and the like.

Avoid FFAs like the plague!
Before doing any paid submissions do a rough ROI (return on investment) calculation.
All sites can be submitted to DMOZ for free, non-commercail sites can be submitted for free to Yahoo! and to zeal. There are two other general purpose directories that many try to get included in: JoeAnt and GoGuides, do a site search on them. I feel that they have limited current value but the potential to be more useful in the future.
Do not forget specialized directories, either regional or topical (or both) depending on your site.
