I think that you can count on spending between 5-10 hours researching into the competition, keywords, and company.
Beyond that, the monthly maintenance fee depends on your pricing structure and how you have it set up. Some companies choose to have no, or a very small, set up fee, and instead have larger monthly fees. On the other hand, some companies choose to have a larger set up fee, which usually covers all initial optimization and production, with a smaller monthly maintenance fee. In this situation, the monthly maintenance fee tends to fall between 10-15% of the initial SEO fee.
I usually spend half a day (10 hrs ;) ) for basic market analysis alone. Usually analyse a simple web site ( 30 pages or less of plain vanilla HTML) in 2 hrs. 'Maintenance' is very relative to market competition.
First getting on top is already a big chunk, staying there is another. No average for 'maintenance' for me. You never can predict how competition will react.
Some SEOs have prices on their websites so you could check those out (not going to name names though). However, most do not as they know that the cost will vary because of the factors above.
A lot of SEO specialists will have an hourly fee for consultancy but also offer a packaged or full solution fee that is slighly lower if looked at on a per hour basis.
Other SEO specialists don't charge anything up front, preferring to charge for the traffic that is referred as a result of the campaign.
There are numerous ways of positioning your pricing. The method you choose will mostly depend on what market segment you're after and what that market are willing to pay and/or how well you are able to sell your services.
