Need PHP polling/survey script will the following features...

I'm looking for a php polling or survey script that has the following features. So far, I haven't been able to find any.

- add/delete/edit/archive polls/surveys
- have multiple questions per poll/survey
- have different ways of displaying answers for different questions (dropdown, radio, etc.)
- display results with image bars and percentages.
- be able to put the poll/results directly onto my current webpage.
- basically, I want a system somewhat like Ballot-Box.net, but with unlimited questions and answers.

Any help would be appreciated.

PHP Nuke and other CMS and forums like VBulletin or PHPBB2 have such modules. You need customization ( workout ) of existing code or start from scratch.
