Dynamic Multiple Entry Form in PHP

Hi
I am new to PHP and trying to create a form which will allow the user to submit data which has two fields. after entering the first data there should be two buttons. One is ofcourse 'Submit' Button the another is 'Add Another'. If the user click on 'Add Another' Another text field shoul apper below to anter the second row. when he finish he click on the 'Submit' button and the php should insert all rows at ones..
Databese is MySQL.
