Good PHP form processor?

I need to build a fairly complex submission form for a client and frankly, don't feel like coding it myself. Why reinvent the wheel?

I need it to:
1. Email her the data.
2. Handle file uploads and/or email attachments
3. Submit the data to a MySQL DB.

Obviously, security is paramount. Form validation, captcha, etc. Also, the script should NOT require an email address from the submitter (although I can probably change that in the coding). These will be kids, many of them under 13, and there are legal issues involved with asking for email info from kids.

I've been roaming through Hotscripts, but thought I'd ask and see if anyone here has a favorite form processor they can recommend. Free would be nice, but a commercial script (up to $30 U.S.) would be fine.
