How to make design of forms in correct way if we have business and non-business clients on shopping websites.

Will be there one input for Company name is not an issue. Problem is when there is added company name and input for City. Will be this city for company or city of person? Problem is when there is added company name and input for City. Will be this city for company or city of person?

Is the correct way to build non-business form like:
- A Business (commercial use)
- An Organization (non-commercial or non-profit use)
- An Individual (personal use)

or just one as top usability skills shows us simple forms for registration.
