I am using MyQry to help me set up a query for users of my web site. The advanced template can be used as is to set up one text search - like in a search engine - of all fields and display the results.

I want to set up a form like the template in MyQry and then pass the text entered in each field to another page which displays the resulting records. For instance, the user might enter a first and last name for a search.

I am not an experienced MySQL user and I like programs that generate my code for me. Any suggestions on how us MyQry to do this multiple search?

There don't seem to be any MyQry users in this forum :) Nor too many that use programs that generate code for them. Typically you'll find folks writing their own code here. If you are willing to roll up your sleeves and learn a bit, you'll get plenty of guidance and assistance though! Do you have a form setup yet? How about your database table?
