Suggestions Needed for Managing Webforms on Multiple Websites

Does anyone know of a server application that can create and manage webforms on multiple sites, store data in a database and email results?

This would be something we would store on our own dedicated server - not an externally hosted solution.

I've been looking around a lot and I'm not finding much...

You should not expect an open source or even paid software to fulfill your all custom requirements. Therefore I always suggest people to make their own applications wherever possible.

Still you can search sourceforge for open source and google for paid softwares. I know a paid one but cannot post its link due to forum charter.
