upload images along with other info

I'm trying to build a form that has you upload images along with pieces of info (name, phone, etc.). The client sent a link to a site that does what she wants hers to do for the most part. The form that comes up after doing that is what i'm being asked to mimic and since i have very little knowledge of php i have no idea where to begin with so much as finding a tutorial for it. can anyone help?
