Getting css and php to interact to give headers / footers

Hi all,

I have built a php / mysql system for a company ordering system. The problem has come where i am trying to generate the delivery note.

For every page that the delivery note spreads over i need to attach the company logo + message to the top of the page, and the company contact details to the bottom of the page.

Anyone know how to do this?

Is this for print or online?

If online, why not just use an include file?
