Oracle ERROR--Please Help

ORA-12640-Please help, i need this ASAP

When I tried the connection to oracle I got error

ORA-12640: Authentication Adapter Initialization Failed,

I wonder what happenned, this happens after i tried to migrate to PHP 5 with the help of instant client(it worked out fine in PHP 4).
But when I revert it back to PHP 4 it still didn't work HELP ME, What Should I Do
