Created a form on a web page and is working. However, I am unable to figure out how to get the submitted name in the () as indicated below. Another form does show who submitted the form.

() on Wednesday, February 21, 2018 at 14:38:16

According to the documentation [scriptarchive.com], the two input fields that fill the From: header are "email" and "realname", so make sure you have at least one of those. Also please make sure it's all set up [scriptarchive.com] correctly to avoid abuse of your form by spammers.
