need to add questionnaire with paypal purchase

i added a bunch of paypal links to my site so people can purchase different levels of membership.

however, when someone applies for membership, i also need to require that they fill out a short questionnaire about their business.

how do i work the questionnaire into the flow? should it come before or after the paypal purchase? whats the best and easiest way to set it up? thanks!

well i need to have the people fill out the questionnaire, which includes some legalities, first so for the sake of discussion lets say that's more important and needs to come first. so how do i build it so that when someone clicks the "Add to Cart" paypal button, then go to a questionnaire, then go to the paypal checkout screen?
