There is a personal hiking site (not mine) that has a few links to its index page. One of the internal pages was, for many years, the only online source for an important list of peaks. That internal page got many, many more links than the index page. Hence higher PR.
That is unusual, but as the example shows, can occur.
Until now Google awarded the index page a PageRank equal to or greater than the highest ranking page on the site. i.e. Pagerank is "Page" rank except in the case of the index page.
This is the first time I have seen an index page with a lower pagerank than another file on the site
