At first I used mydomain.com/PAGE.htm and then I changed this page to mydomain.com/page.htm. Now I have two pages in SERPs and page.htm is a duplicate of PAGE.htm.
How can I get rid of mydomain.com/PAGE.htm in SERPs.

If you have already deleted /PAGE.htm from your server it should disappear from the SERPS soon. If you didn't delete /PAGE.htm yet, I suggest you do that ASAP and then use a 301 to redirect to /page.htm.

Actually, the PAGE.htm was replaced and thus deleted over six months ago. Both pages now are fully listed in Google. The PAGE.htm is with a cache dated Feb 2004. How long Google will delist the old page? Any idea?
Is there any limitation on 301 redirect for a domain?
If I use a lot of 301 redirect, will be any effect on page ranking?
