How do I out find out if my site was "banned"?

Hi,

How do I out find out if one of my site's was "banned" from google?

I do not why it would be, but I was always ranked about 12th for my 3 most important keywords....then about 3 weeks ago I was in the #1 postion up until 3-5 days ago.

And now its not even listed anywere.

If it was "banned" or "penalized"...is their anything I can do to get it re-indexed?
