What does it tell you

When i do a search for related:mydomain.com it brings several related sites but also bring up quite a few that are a million miles apart

How does it work out what is related and is there something to be learn't from the results. is it based on who links to you, body content?

From my experience, no research done on that, it shows sites from your linking neighbourhood. Sometimes it's good, sometimes not...

If site A links to site B and site C then searching for related sites of site B will show site C. Whereas the link: commando on the home page of site B will show the page on site A that linked. For both commands ('related:' and 'link:') the PageRank must be high enough (around PR4) to give results.

Does this also mean that a backlink to boost your own PR must be PR4 or higher, or only that it will not show up in the "link:website" results unless it is PR4?
