The problem doesn't seem to be related to a specific data centre (i.e. it can occur at all of them). The problem also arises for a search of allinanchor, allintitle and allintext of keyword1. However, it seems that other keywords or combination of keywords are not affected.
Anybody else seeing a similar behaviour?

I've been tracking the top 20 sites in my genre/serps, at the ones that are having the "disappearing index" problem have all failed to show any fresh tags in the recent weeks.
It baffles me why freshbot visits some pr4 sites on a daily basis, yet ignores much larger/older/higher PR sites in it's daily drive-by.
In fact, there are several pr4 sites that were (before becoming MIA) below my listing that have received freshtags, yet my PR6 site has not received a fresh tag in months.
So...here's a question:
What is the best/fastest/easiest way to encourage ole' freshbot to start coming around again? Since I have a PR6, and googlebot(s) crawl my site almost on a daily basis, it can't be tooo hard, right?
You would be much better off if you started a separate post about attracting FreshBot.
Index page in not coming at all even after many refreshes. It is not coming even 2nd and 3nd server of google.
Main 2 word keyphrase appearing fine when searching on google.co.uk and filtering to UK only - across all datacentres + appearing on all but 1 datacentre when search for "kekword1 keyword2 UK" and not filtered to UK only (I put the one missing one down to fluxing between datacentres that has been going on for months?)
Anyway, when I search for "keyword1 keyword2" on all datacentres, NOT filtered to UK, index page is nowhere!
This doesn't cause me a problem from a traffic point of view as the top 50-60 results on a .com search are all US sites, so people looking for my product will insert a UK or search at .co.uk filtered.
But - something is obviously not right and this doesn't give a massive amount of confidence that something else may occur and drop my index from .co.uk filtered searches.
The other strange thing is that this is exactly the opposite as to what was happening during the Dom/Esme twilight zone. Then this was loosing me traffic, but I'm still concerned that things aren't back to 100% normal
Anyone else experiencing this? GoogleGuy - does this interest you or are you aware of why this may be occuring?
My PR6 site is still MIA for main search phrase, yet still appear at all data-centers except www-fi when company name is searched for.
As "luck" would have it, however, I've attained the #1 spot on evey other major engine (altavista, inktomi, teoma/ask, wisenut, etc.), which has brought back about 50% of the traffic I've lost due to the "missing index page" phenom at google. (Kind of sad...a #1 at ALL those engines is ony worth 50% of the traffic a #4-5 spot brought at google)
I was hoping the rolling update wopuld have brought some changes this weekend...but no luck.

Report #1: kp, the report you did seems to work for me now. I'll keep an eye on it though.
Report #2: for the other report: I see the index page, just indented under the other subpage result. I noticed a large amount of links to the subpage, so I'm not surprised it shows atop the index page in this case. In some cases (e.g. an ODP entry points to a subpage instead of the root page), the subpage can have more pagerank than the index page.
I'm getting ready to close this out, but if anyone else sees something strange, do the spam report with "notindexpage" in the description if you think there's a problem with subpages vs. index pages..
Which answer for me? Report #1 or Report #2?
It is still pr 3 but wont show up on resaults for two weeks now.

Another little but very important question, although My website's index page has started appearing again (as you suggested) in google, I am not sure why my position went down so much in search results for almost all keywords. For a keyword, a week ago, I was in position 7, but now it has decreased to 52. I am really surprised to see this because I was expecting improvements as I had gained many back links from our new customers, which helped the page rank of my site, but discouraged the position in search results. Could you please give me some information in this direction.
We hope this is temporary. Any insight into this will be immensely appreciated.

I have just sent a spam report to report an Index page ranking problem
I have inserted "notindexpage - Webmasterworld - GoogleGuy - UK Web Guy" within the text box options of the form.
did you find my spam report? let me know if you need me to send it again.
This makes no sense as my site appears below sites it was appearing above on the .com. Any ideas anyone?
Your site isn't on a US IP address is it? - This would explain why a .co.uk filter knocked it out
Hi hope GoogleGuy has seen my spam report as the fact that something as obvious as this is happening is an indication that the index is not 100%.
The problem site does not disappear from the results when the .co.uk filter is applied, it just appears lower in the results than on a .com search. Do you still think the problem is caused a US IP. I am based in the UK and the site is aimed at a UK audience, the site is only on a US server as the UK hosting company has its servers in the US.
Up until three months ago there were no problems.
If you are still having a problem with your index page vanishing, follow GoogleGuy's instructions and submit a report. I'm confident he will look into any remaining complaints if he gets them.
