Probably they just want first hand information as to how many publishers are turning to other options from adsense. :)
I just wanted to say that I feel it will (filling of survey) least affect your adsense account / adsense earnings in any way. Google is just trying this PR exercise to get the first hand information from Publishers and may be confirm their own findings on this.
Now that I have filled up the survey I will keep posting whether it makes any effect on my adsense earnings.(Positive or negative)
I'm not saying that anything will actually come from it (although I certainly hope some changes will). But if Google gives you a link to provide direct feedback about the program and you choose not to make use of it, then I think that should preclude you from any future public criticism of the program.

Asking what revenue share I make from their competitors is something completely different than asking for feedback for their program in my book. Yes, I bailed out at the same point ken_b did.
As far as the usefulness of that question, if Google finds that their market share is decreasing to their competitors, one would hope that they would be more willing to listen to what the survey takers are saying to improve the program and regain market share.
Adsense will add features or make other changes to the program when the adsense team has determined, by whatever means they use to determine, that it will be beneficial to the program. We asked for some things for years, literally years, and finally got them. Was it because we asked for them? No. It was because adsense had finally determined that it would benefit the adsense program. Not adsense publishers in particular, but the adsense program.
It is a complete insult to my intelligence to hear them say, "Hey, we've listened to your feedback and have now increased the number of channels you can use." Well, whoop-ti-do. Where were you 4 years ago or whenever it was that we first started beeping about it?
Font controls. This is seemingly in direct opposition to their earlier stated mandate of not calling attention to the ads. The font control thing is a joke anyway but did we get it because we asked for it? You think? Well, believe what you want.
I'd take the survey just to vent a little bit but I haven't gotten the link and I'm pretty sure I won't. I'm on record as not being too happy with this program.

Interesting that so many people clamor for changes in AdSense and complain about many aspects of the program, yet so many people don't want to take the effort to respond when Google asks for direct feedback.

From what I see in this thread, it's not so much that people don't want to fill it in, but because of some questions in the survey are preventing them from doing so.
May be is has to do with that little meaningful word called privacy.
Take out such questions and more will fill it in.
If answering "What percent of the revenue from your website(s) comes from Google AdSense?" with 5 selected drop-down options is preventing you from completing the survey, then you might as well pull your tin-foil hat down to your chin and stick your entire head in the sand.
"Privacy issues" has become the new "politically correct."

That's confusing the issue.
The transparency you mention publishers wanting is between those publishers who use AdSense and Google.
In the case of this survey, Google is asking for info about a publishers relationship with OTHER ad networks, etc.
I'm glad to answer questions from Google about my AdSense account and AdSense in general.
I'm NOT interested in giving Google info about my relationship with OTHER ad networks etc, or about my earnings from those relationships.
As for the rest of the comments, some people here don't see anything but Big Brother on one side, while others see a bunch of tin foil hats and conspiracy theories on the other.
For me, I have better things to do than worry about the possibility that some disgruntled employee may leave in the future and somehow remember that my publisher ID matched up with option 2 (or whatever it was) representing Google's revenue share compared to other partners. And that somehow that young man with the photographic memory will decide to somehow use that information (which may or may not have been a true answer to begin with) about revenue share at a certain point in time to create Skynet or somehow otherwise wreck havoc and destruction.
