Site hacked, but hacked pages still showing in Google

My site was hacked about 2 weeks ago and the attacker placed some pages like www.mysite.com/buyillegalstuff

I saw it, cleaned my site and removed these pages (that now return a 404 error), but when I search google for site:mysite.com I can still see these pages ranking.

What should I do?

have you checked your server access log to see if the hacked urls have been crawled by googlebot and gotten a 404 response?

in WMT have you requested the URLs be removed from the index?

I also was hacked recently over 600 pages, took a week to reinidex and clear the cache.

Are you sure you actually cleaned it up 100%? Clever hackers will open up backdoors so they can easily reinfect the site, in case the real webmaster deletes any of their stuff and then they cookie visitors so that only first time visitors see the hacked content, thus tricking webmasters into thinking the site has been cleaned up.

In WMT, if you "fetch as googlebot" on those URLs that the hacker created, what are the results?
