Increase in not followed pages - What is this?

Hi all,

I am getting this new message in GWT recently

Increase in not followed pages
Google detected a significant increase in the number of URLs that we were unable to completely follow.

Check the Crawl Errors page in Webmaster Tools.
Make sure that any redirected URLs point to accessible pages.
Investigate these errors and take appropriate steps to make sure that redirected pages are reachable.

Anyone faced this problem before? Could you please help me to fix this?

I checked all that above, but still confused of knowing the exact problem I am facing right now.

Did you look at your log files to see how Googlebot is behaving on your site?

the crawl errors page in gwt should reveal some type of trend that would explain the message.
