Erroneous Duplicate Title Tags

In our latest Webmaster Tools check we noticed we have a lot of issues with duplicate Title Tags. The only is, the second page for each instance is always a page we haven't created but a flaw in the URL scheme.

EX:

http://www.example.com/widgets/blue
http://www.example.com///////widgets/blue

The first URL is an integral page to our website but we have no idea what the deal is with the second URL. Where does this come from?

There's several ways that could happen.

One is malicious incoming external linking.

The other is an internal linking or redirection error within your site.

What do you see in the "links to your site" and the "internal links" reports in WebmasterTools?

What does running Xenu LinkSleuth, or similar, over your site show you?
