Remove keywords from URL?

I've been reading about Google applying penalties for having keywords in the URL. Is it no longer ideal to have keywords in the URL? Would you include them in new pages?

Sites with more content seem to have a positive semantic effect. I have sites where the URLs and the anchor text both consist of keyword rich phrases but these sites never trip a -950 filter since they have substantial content.

I also have sites with keyword rich URLs and keyword rich anchor text where there is less content, these sites trip this filter immediately not just for certain but for ALL phrases.

I know this is hard to quantify, but what would be considered substantial content?

I am not from USA and my English is poor could you make an example of co-occurring phrase?

They are phases which normally occur (go together) in language.

In your sentence the USA and English are co-occurring phrases...
They naturally happen together in regular every day use of words to some extent.

In my post until this sentence: phrases, sentence, language, words are co-occurring phrases, so are occur and go together, as well as natural, regular, every day use.

It's the same with about anything we talk about. There are naturally some 'phrases' one or more words which are used together when describing a subject or situation.

Car, engine, doors, wheels, tires are co-occurring phrases too.

I'm sure you have the same thing in whatever language you are more fluent in.
