Google De-indexed the inner pages

Recently I have seen there is a huge drop in my daily visits; because there are few pages are got de-index from Google Search and all the datacenters.

My URL structures are

Type/Name/default.htm
Type/Name/1.htm
Type/Name/2.htm
.
.
Type/Name/N.htm

After analysis that I identified “Type/Name/1.htm” got de-index for the entire scenario.
We never made any other changes in those pages (1.htm)...

Why it’s happening? How I can re-index? Can I approach Google Re-Inclusion option?

This looks like it might be a pagination problem. In recent times Google has been weeding out certian kinds of paginated results that have certain characteristics. Have you checked out this thread?

Does all these pages have the same title/description?
