One of my sites has suffered from this problem as well and the impact has been significant. The "penalty" or incorrect classification of images as being adult is very domain related and not image related.
This site has some sections on dating, but also sections on travel, culture, food and architecture. I could almost understand if the dating images were marked as "adult" and the rest weren't but this is not the case.
Just about the only images on the site that pass the "strict" or "default" setting are from Wikipedia or Wikimedia and have been credit as such.
There are still many other pictures from Wikipedia or Wikimedia that only pass the "SafeSearch:off" setting. I can only get Google images to show around 670 images, when there must be more than 3,500 on the site.
I had done some link exchange with some adult dating sites and I have removed my links to them in case this was an issue. The only other offending issue I can think is the use of the phrases like "sexy girls" or "sexy women" in some of the titles and text of pages.
One of the biggest problems with resolving this problem is slow updating of Google image compared to Google standard search.
If anyone has any ideas or solutions I would be glad to hear of them.
