Would it make sense to create folders (i.e. /keyword1-keyword2/index.htm) rather than files (i.e. keyword1-keyword2.html) to rank better in search results?

That's an old trick that used to work on some dead-and-gone search engines, but I don't see it as helping on Google. Now if you have a directory with many pages in it, that's another story.

it might help if you're into tagging and microformats. because the last word in the url acts as the tag. it doesn't apply if it's got a file extension on the end though.

Don't know about ranking better but it's a good way to avoid changing urls in the future in case you decide to change file extensions.
