My site was and has been verified for a few months now, then today it is no longer and requesting me to verify it. Is this a "bug in the system" or will this be a repeating issue?

Google will re-verify your site at various intervals to make sure the site didn't change hands. See these comments from Google's Vaneesa Fox:
