Checking my logs this morning, I came across a referral from google/custom.
It's plain .com/custom and also country specific like .ca/custom or .co.za/custom.
Anybody seen this or have any idea what's happening there?

Probably someone with a custom search engine has added your site to their list of sites to search. I've seen a few of these in my logs (and I've got a couple of custom search engines myself) I'd kinda like to know whose custom search engines my sites are in, but as far as I can tell, there's no easy way to determine that.
