When I do a search for inurl: I see 46 pages of our site that are in the regular index and are listed in G DB.
However, when I do a search for site: I see my entire site (each and ever page) listed as supplemental results.
What's going on?

Interesting - are the cache dates the same?

Yes. They have the same cache date.

Then I'm very curious -- are the results from the same Google IP address?

Yes. The exact same IP address.
Under site: my entire site is in supplemental.
Under inurl: only 46 pages are in the regular index, and we do have traffic to the in url pages only.

Then I'm very curious too ;-)
Whats the "real" number of pages of your site?

Google shows 272 pages under site: and as mentioned G shows them all as supplemental results. (we have a lot more pages, don't have an exact figure)
inurl: G shows 46 pages in regular index.

- Would you be kind to mention the datacenter IP you are looking at?
- Does site: operator shows your home page at top of listings?

IP Address 64.233.167.104.
No. Our home page is not first under site: it is first inurl: though.

It's been like that for at least a year with my site and it doesn't matter which IP. Only now since I just checked it, my inurl only shows 8 pages while my site: shows 43,000 (all except 53 are supplemental). I quit trying to figure it out long ago.

It seems that sites which are suffering of the supplementals issue might show some odd results as you mentioned for both site: and inurl: operators.
