Assuming that I have reverse DNS look-up enabled on my webserver, can I always expect Google's crawler IP to translate to "GoogleBot"? In other words, are there ever any instances, to anyone's knowledge, where Google's Bot will not resolve to a "GoogleBot" DNS name, and just show up as a plain old IP address in the web log?

No

Because G looks for cloakers and other such things and does not publically announce all of it's crawlers.
