This page says:
'If your site has violated our webmaster guidelines, and you've made changes to it so that it meets our guidelines, you can request reinclusion and we'll evaluate your site.
In addition, if you recently purchased a domain that you think may have violated our guidelines before you owned it, you can use the reinclusion request form to let us know that you recently acquired the site and that it now adheres to the guidelines.
To request reinclusion, log in to Google Sitemaps, choose the "Request reinclusion" link, and follow the steps outlined there.'
But when I log into my Google Sitemaps page, I find no link called 'Request Reinclusion'. Has Google abolished the 'Request Reinclusion' form? Or am I looking for it in the wrong place?
Any advice on how to find the Request Reinclusion form would be very gratefully received.
Welcome to WebmasterWorld.
As netmeg mentioned correctly, you find the link for submitting a reinclusion request just over the green bar, under the link which reads:
+ Tools
you click the above link, and see:

And you may wish to read Matt Cutts "evergreen" :

Good luck!
