What would be nice is to be able to tell Google the relationship between pages.
For a site with index.php?id=x, all pages have a parent child relationship but there's little way of expressing this.

The only relationship google understands in the link. The form of the url makes no difference, be it index.php?id=x or \keyword1\keyword2\keyword3.html. Argueably the later better describes the page, but as far as I understand the URL structure does not imply any relationships.

Can sitemaps imply some sort of hierarchical structure?

Each section index page links to every page of that section.

Every page of that section links back to the index page for that section.

The root index page links to every section index.

Every section index links back to the root index page.

.

The section index pages might all link to each other.

The pages within one section might all link to each other.

The pages within one section might link to all of the section index pages.

The pages within any section will all link back to the root index page.

There will be very little inter-linking between the sub-pages in one section and the sub-pages in some other section (maye a few "related" or "upsell" links).

That is your heirarchy.
