looks like some kind of mortar and pestle type thing

put your stuff in the bottom and you can grind it

What does the top look like? Is it just a hinge or does it carry on out of the pic?

Does the long thin needle like thing have an eye at the other end, that we cannot see in the picture, for threading something through?

It looks like it is made from bone or ivory?

Well, now it's obvious!

It's a toothpick holder.

The toothpick is clearly shown and the food that you get on the toothpick is stored in the bowl-shaped reservoir for later consumption.
