i canīt get my search funktion to work

i canīt serch in a form and find it in my databsae

how does i make a search funktion that makes it possible to have a html form whit 5 textareaīs and if i only want to search for example name i just write in name then everything that is LIKE that i have write in my textareaīs shall be writed on my pages

i have this to files
and i dont know if it is possible to make this to work or if i shall do it inanother way

i get this wrong message now
