Opt-in -email just stopped

Hello Everyone - some help please

I've a user sign-up form that sends data to a MySQL database. Once a user signs up, an opt-in email is sent. This has been working fine, but it has just stopped? I've not changed anything.

Any ideas?

has your host changed anything?

What exactly has stopped, just the sending of the email?

have you tested the script to see if you can get one sent to yourself?
