PHP Forms with file uploads

Anyone know of some good software with a form generator?

Hi All, I've been messing about trying to get PHP forms on my site so customers can send me information and image files, but much of the software I've tried out has either not had the option of file uploads, or limits them to one image. Anyone know of a good "off the shelf" solution?

Take a look on hotscripts.com at the file upload systems, you should find something there.

There's a fairly steep learning curve involved but it's worth it in my opinion.
