registration form help!

Could someone please help me write a script to insert the info:
Username(user-min 5 characters)
Password(pass-min 5 characters)
Re-type(re-pass-same as pass to enter info)
E-mail(email)
into a mysql database.

I have tried variations, but as yet cannot quite get it right.

show us what you have and we can help walk you through it. or post the codes that are giving you problems. i dont imagine many people are gunna write it for ya, cause WW likes to teach :)
